Assessment Type
Determine what assessment type you are requesting. The list of assessment types is provided below.
After You Submit Your Completed Form
We will review the booking form and determine if more information is needed or we can proceed with adding you to our wait list. You will hear from us within 1 week to confirm your booking form has been received. Once your child's name has come up on the waiting list and is ready for scheduling, we will contact you and send out digital intake forms, consent forms, questionnaires and may request other reports or information from you. We request that these forms be returned to us within 2 weeks, so that we can schedule an assessment date.
Types of Assessments
Types of Assessments offered:
· Psychoeducational Assessments (ages 7 and older)
· Developmental Assessment (ages 2 to 6)
· Autism Assessments for ages 6+ and up
· Autism Assessment for ages 2-5
· CLBC Eligibility Assessment:
· Completion of forms (e.g., disability tax credit, CLBC) – billed at $180/hour (pro-rated)
