Booking Form

Complete the Booking Form and email, fax or mail it to our office.

After You Submit Your Completed Form

We will review the form and determine if more information is needed or we can proceed with adding you to our wait list. We will contact you and send out intake forms, consent forms, questionnaires and may request other reports or information from you. You should receive these within 2 weeks of submitting your booking form. Once the intake forms and information are collected and received, then we will contact you to schedule a specific assessment date.
